In my 3e campaign we didn't use minis, which almost entirely
eliminated AoOs. They became more a matter of my judgement and decree.
It aso eliminated the value of quite a lot of feats and nerfed the
rogue something rotten, though as he was a craven coward of the
yellowest stripe I doubt it really made that much difference.
Personally, I don't see AoOs as much of a problem. If you're using
minis they work OK. Without them you need some way for the grunts to
control some ground, and that way is likely to be even more complex.
Not that grunts can control much ground without a reach weapon and
Combat Reflexes, but never mind that.
This calls for a feat:
Interceptor (Combat)
You can quickly react to anyone trying to get past you.
Prerequisites: Combat Reflexes, Spring Attack
Benefit: As an immediate action you can take a 5' step when an
opponent moves past you if this step would place them into your
threatened area. You may then perform an AoO on them as normal if they
move out of that square. This 5' step counts as your 5' step for the
coming round.
